silver gear knob tops used to be about £5 from Peugeot, not sure if you can still get them though

Ive got a gear knob at home with a good silver top. The leather has seen better days but you can have it for the price of postage if you like?

Can I just change the top or does it have to be the whole thing?

You can pop the top out and glue a new one in.
